Health History Form - Must be filled out prior to first non lash lift appointment.
Pre Appointment Form - Must be filled out prior to every appointment that doesn't have a specific consent form.
Chemical Peel Appointment Form - Must be filled out prior to a chemcial peel appointment.
Waxing Appointment Form - Must be filled out prior to a waxing appointment.
